There are two kinds of QR codes. A static QR code holds the URL itself in its pattern. A dynamic QR code holds a short address that belongs to a service, and that service’s server sends people on to the real address.

Many QR sites make dynamic codes by default. They show you scan statistics, and when the trial period ends the short address stops working. That is why a QR code printed on a flyer stops working one day.
